sERVICE DELIVERY

The Restorative Care Lodge’s community-based service is delivered under the guidance of the Service Manager alongside registered nurses, support workers and the individual’s community clinical team. The service is designed to support recovery, reduce reoffending risk and promote safe reintegration to the community in line with the Transforming Care Agenda and the NHS long-term plan.
Nurses and support workers work collaboratively to support the service users’ 24/7, working closely with the service user’s Multi-Disciplinary Team and families. This ensures continuity of care whilst managing clinical risk in a least restrictive setting ultimately offering commissioners a robust and sustainable alternative to long-term inpatient care. Staffing ratios are tailored to individual needs with provision for enhanced support where required
Service users are offered structure and routine in a comfortable homely setting as part of the rehabilitative process with private bedrooms, communal areas and gardens.

integrated support plans

Service user support plans are dynamic documents that are reviewed regularly and updated to reflect the evolving needs and preferences of the individual; with the service user remaining at the heart of the plan. Our least restrictive support plans encompass the following:

-
Patient background including cultural needs
-
Mental health support
-
Physical health needs
-
Risk management
-
Daily living skills
-
Social inclusion
-
Communication skills
-
Outcome and recovery goals

Relapse prevention protocol is written into each individual service user's support plan along with Early Warning Signs and Advanced Statements, should a service relapse. Joint crisis planning, with MDT and family will be conducted. This is in line with the NICE crisis planning QS14 standard.
Restorative Care Lodge will retain all relevant contacts in event of crisis as part of the support plan.
relational security

At Restorative Care Lodge we embed relational security principles into our service to allow for safe, therapeutic, rehabilitative environment and continuity of care. Our staff are trained to understand the key principles of relational security particularly in relation to our core values to support service users through building relationships rather than following ‘rules’. This allows our staff to focus on promoting independence, safety and recovery. Utilisation of the ‘See, Think, Act Framework’, dynamic risk formulation and weekly reflective meetings allow us to achieve the best possible outcomes for our service users.
